Evaluations
Evaluations are conducted for a variety of reasons and can be a valuable tool in helping your child succeed in school.
I am able to conduct assessments for learning disabilities, dyslexia, ADHD, autism, social-emotional functioning, behavioral concerns, and strengths and weaknesses as related to learning or vocation for both children and adults.
Areas of assessment may include:
Psychometric Testing: This includes testing of cognitive abilities, academic achievement, and psychological processing. These tests measure a child's reasoning skills, intellectual ability, memory, attention, academic functioning in the areas of reading, writing, math, and oral language, and how a child perceives and processing information through hearing, sight, and touch.
Social-Emotional Functioning/Behavior: These tests will indicate how a child feels about him/herself, gets along with others, and how they behave within the home and school environments.
Executive Functions: These tests measure attention, planning, and mental flexibility.
Adaptive Behavior: These tests indicate how your child takes care of personal wants and needs on a daily basis.
Evaluations may be conducted in person at a quiet public location (i.e. library) or virtually depending on preference.
ADHD or Dyslexia Diagnostic Evaluation
$800
These assessments yield information in regards to a diagnosis of ADHD or Dyslexia. The assessment may include (depending on diagnostic consideration) cognitive level, phonological processing assessments, standardized measures of attention, social-emotional functioning, and/or academic performance in the areas of reading and writing.
Giftedness Evaluation/IQ Testing
$400
This type of evaluation is meant to provide families with their child's Intelligence Quotient (IQ) score. This assessment will yield an IQ score and percentile rank and information in regards to whether a child should be considered for gifted programs. This evaluation is most often used for entrance into private schools requiring IQ testing as part of their admission process.
Psychoeducational Evaluation
Starting at $150/hour
Psychoeducational Assessments is conducted in the areas of cognitive abilities, psychological processing skills, social-emotional functioning, academic ability, review of relevant records; interviews with parents, student, and teachers; and rating scales completed by relevant sources. These assessments include diagnostic information in regards to learning disabilities and/or behavioral/social-emotional difficulties. All comprehensive evaluations result in a formal written report with recommendations for the client, parent, or educators.
Independent Educational Evaluation
$3500-4500
Independent Educational Evaluations are typically requested by a school district, attorney, or parent requesting an evaluation at public expense. In comparison to private psychoeducational evaluations, these evaluations require extensive review of school records, travel, and interaction with IEP teams, attorneys, and attendance at Due Process hearings. Independent Educational Evaluations include review of relevant records; comprehensive review of educational records, interviews with parents, student, teachers; multiple observations within the testing environment and educational setting, rating scales completed by relevant sources, participation in relevant IEP meetings (maximum 3 hours). Travel time is also included in the fee. Any additional time required to participate in a due process hearing or additional IEP meetings will be billed at a negotiated hourly rate.
Consultation
Services can be provided virtually or in-person.
Psychoeducational Evaluation Report Review
Starting at $100/hour
I can review psychoeducational evaluation reports conducted through the school, medical facility, or other provider and clarify information and provide recommendations based on provided information. I can consult on questions or supports that you can bring to your child's IEP team. I do not attend the meetings.
Educational Consultation Services
Starting at $100/hour
I can consult on a variety of concerns you may have regarding your child's education or social-emotional functioning. This consultation does not contain an evaluation but I am able to provide recommendations and/or guidance for steps you may need to take within the school system to benefit your child. I can consult regarding behavior concerns within the home and provide practical strategies to use with your child.
Mental Health Services
Services are provided one on one. At this time, counseling services are provided virtually.
Counseling and/or Mental Health Screening Evaluations
Starting at $100/hour
Counseling services can be provided for a variety of behavioral and social-emotional concerns. Each new client will begin with an intake meeting and screening evaluation which will take approximately 1-2 hours depending on needs. All counseling services are provided individually and target specific goals throughout treatment. Through a variety of techniques, including cognitive-behavioral therapy and solution-focused therapy, counseling services can be provided to address needs in the areas of anxiety, impulsivity, aggression, attention/focus, social skills, and depression. Sessions are typically 50 minutes, with an additional 10 minutes for parent consult (if a minor under 12), treatment notes and progress monitoring.
Insurance
At this time, Lindsay Jack, LEP is not aligned with any health insurance providers. This is a pay for service practice.
Payment
I accept cash, check and credit cards through Square.
For evaluations half of the fee is required at the first testing session and the second half is required at the consultation session.
For counseling, payment is required at the end of each session.
Cancellation Policy
If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you may be charged for the full rate of the session.
